Power of Attorney Forms


 

A Power of Attorney form is a legal document in which you give another person legal authority to act on your behalf. The person who signs a Power of Attorney form is called the principal and the person you give authority to act on your behalf is called your agent or attorney-in-fact. A principal can give an agent broad legal authority, or very limited authority in a Power of Attorney form. A Power of Attorney form is frequently used to help in the event of illness or disability, or for financial transactions where the principal cannot be present to sign the necessary legal documents.

After you have drawn up your Power of Attorney form, your agent should keep the Power of Attorney form in a secure location where it will be readily available when needed for the agent to act in the event of an emergency. When using the Power of Attorney, the agent should sign for the principal in the following manner:
(principal's name) by (agent's signature) as Agent, or
(agent's signature) as Agent for (principal's name).
Furthermore, the agent should always retain the original Power of Attorney and give copies to persons dealt with.
