A legal name change is obtained by completing a simple legal procedure at your local county courthouse. The usual requirements are that you file two to three forms at the courthouse and attend a hearing. The first step is to complete a legal form titled "Petition for Change of Name". A Petition for Change of Name is a legal document that requests the court to grant you a legal name change. There are a small number of questions that you complete on your Petition, such as your present name and address, the name that you are requesting the court to change your name to, etc. The next step is to complete a legal form titled "Final Judgment of Change of Name". The Final Judgment of Change of Name is a legal document that you complete and provide to the court for the judge to sign. As long as the court/judge does not think that you are trying to attempt to commit some type of fraud or hide your identity by changing your name, you will usually be granted a legal name change after only two trips to the courthouse. When changing the name of a minor child, there are usually a couple of more forms involved. A “Consent” form is usually required that the other natural parent signs and the minor child will also need to sign a “Consent” form if he/she is 14 years of age or older.
